More than likey this is not a IM problem but a CGI interface problem.
You are spefying the appropritae CGI header to tell the server and calling browser what type of file (image) you are outputing.
Perhaps you should go to a basic PHP image generator and compre what it is doing with what you are doing.
Finally, check with the PHP help forums.
